Is swelling normal 7 days after breast augmentation under the muscle?

Patients often ask this question and therefore I find it important to let you know what to expect during our consultation and in the first visit after surgery.

Swelling after breast augmentation can be caused by:

1) Normal swelling/healing process

2) Bleeding- very rare

3) Fluid around the implant

4) Infection

There is always swelling after breast augmentation and this swelling usually is at its worst day 3-5. Swelling is also more pronounced above the nipple and areola when the implant is placed under the muscle. At 7 days after surgery, swelling that is realtively symmetric without signicant bruising and acceptable discomfort is highly indicative of normal post operative swelling. You should be seing the swelling subsiding over the next week. Continue to minimize your activity as instructed to prevent additional swelling from occuring. We will see you several times during the first few weeks after your surgery to make sure everything is going as planned. If you are ever concerned, however, please don’t hesitate to give us a call.

Our breast reconstruction patients are so appreciative, and we love taking care of them!

I am in my early 40′s, BRACA positive, and have been living under the dark cloud of my fate since 2003. Every 3 mos I have been alternating the Diagnostic Mammogram, than the Bilateral MRI. I have been fortunate to of had my Breast Cancer caught very early in 2003 and have had 3 lumpectomies since. In May of 2010, 3 of my Doctors strongly suggested I have a Bilateral Mastectomy. I have had an ongoing issue in my right breast for the past 11 mos. and my emotions along with my Doctors care and concerns were nearing the end. My fate ran out and here I was in a daze having to choose a Plastic Surgeon to assist my surgeon during my proceedure. I was given a sheet a paper with a few Plastic Surgeons listed and was told to choose so we could proceed forward.I was still in shock, numb and felt like a walking zombie. I was definately not in any mental shape to make such an important decision. With the list in hand, and some referrals from friends, I set up 3 appointments with 3 Plastic Surgeons in the hopes that I would “just know” when I met the right one. Dr. Sternberg was my 1st appointment. Due to my Surgeons recommendations and another good feeling I had prior to meeting him, I felt he would be the perfect one to start off with.I just wanted to tell a little of my story so you can truly understand what I am about to disclose.I made my appointment with such a “Professional and Caring” member of his team. Ok, now onto the appointment. I arrived at Dr. Sternberg’s office and what a warm and loving feeling right from entering the front door. I thought to myself, ok… when does the bomb drop??? Well, my wait was only about 5 minutes and I was placed in a room that was filled with the very same feeling I received when entering the office. Dr. Sternberg entered the room so promptly after I arrived. Well, the rest is history. If we only had more Doctors with the care, concern, bedside manner and knowledge as Dr. Sternberg, I feel that many would be more proactive in their own or a loved ones health. Dr. Sternberg, by far, is the most Amazing, Caring and Assuring Doctor. I am SO BLESSED to of had him assist my Surgeon in this very difficult time of my life. I cancelled the other appointments I had made with the other 2 Plastic Surgeons and before I left his office that very first day, I told him he was the one! I ONLY went to him and chose him immediately due to that “right feeling” I so was hoping I would get.His office was the HUGE hug and assurance I needed at this very trying time of my life. I am not a Doctor hopper and feel very blessed to have such great relationships and care from my Doctors. Dr. Sternberg is at the TOP of my list and words can never explain how he helped make such a scary time for me…. OK. Kristen, Robin and Dr. Sternberg, Many Thanks!!! I am so fortunate to have all of you walk this Journey with me. I adore all of you, and though we are at the 2nd stage of this long Journey, I just know it is going to be ok… I hold you all so dear to my heart!!!If any of you are reading this because you are in a similar situation as mine, or are just at that stage to choose the right fit for you, let me say…. Materials used in illegal implants and augmentation procedures can cause serious lung and brain damage. May also lead to serious infections.

Hardware store-grade silicone used by unqualified practitioners for buttock and breast implants (as recently happened in Miami) can dissolve in the body. Despite media reports of deaths and injuries, the incidence of illicit procedures may be rising. “We’ve been hearing about this,” said American Society of Plastic Surgeons President Malcolm Roth. “There are ‘pumping parties,’ involving high-volume injections to fill up the face, lips, cheekbones, chin or breast. Often it’s buttock enhancement and often it’s not sterile.” The ASPS is reminding patients to check and understand providers’ credentials before seeking cosmetic procedures. Any surgical or invasive procedure performed at a “party” or at someone’s home is likely illegal and dangerous. Please know what you are getting into and always ask to see credentials or a valid medical license.

Study: Nipple sparing mastectomy safe for breast cancer patients

A study in the journal Plastic and Reconstructive Surgery revealed that nipple sparing mastectomy (when the breast tissue is removed with the entire breast skin and nipple left intact and an implant or expander is placed at the time of the surgery) is a safe option for some women with breast cancer. Researchers assessed the outcomes of 169 procedures conducted at Georgetown University Hospital and found no recurrence of cancer and no new cases on average for two and a half years following the surgery. However, researchers warned that not all women are eligible for the procedure.

This procedure can be performed in the right setting with a small non-invasive cancer or when prophylactic mastectomies are planned due to the BRCA gene trait. Can Plastic Surgery have a bad result?

It is always a touchy subject to discuss bad surgical outcomes. Surgery can have a bad outcome for many reasons:

1.Adverse effects can happen to anyone, regardless of the surgery or doctor. Things like infections, drug reactions, implant rejections or skin necrosis (wound healing problems) can happen to anyone. I always say that if you do not want any risk of complications then don’t have surgery.
2.Adverse effects can happen because of poor patient selection. Many patients actually do not help themselves toward a positive outcome. These type of patients include smokers, drug abusers and patients who are morbidly obese.
3.The main reason patients love to complain (and lawyers love to sue) is poor outcomes as a result of a physician’s treatment.

Now, most doctors are brilliant hard-working caring surgeons who try their best. In most circumstances everything turns out great, but sometimes treatment plans are sound but judgement in the amount of skin excised or the angle of pull is off. These are things that can easily be corrected by the attending physician.

Often, patients expect perfection immediately after surgery and then transfer their disappointment into anger towards their doctor. In that case, they will often see another surgeon to perform the minor corrections even though their first doctor is capable of the correction.

Other outcomes may, in a small group, be the result of of the doctor not demonstrating a caring attitude toward the patient. As a result, treatment plans may not reflect that which is most beneficial for the patient. These are the cases that give doctors the worst problems as a group. It is sort of like letting a bad apple spoil the entire bushel.

In our practice, we see patients that fit into all of these groups. We look at the treatment with understanding and compassion. For those that we know we can help (those that have reasonable expectations and are willing to help themselves), we finish with a well-tailored plan for a successful outcome.

“Can I have Plastic Surgery if I have diabetes or high blood pressure?”

The medical conditions you describe place you at some increased risk but do not necessarily mean that you can’t have a tummy tuck. It is important to discuss all of your medical conditions with both your plastic surgeon and your medical doctor.

It will be important to have your diabetes, blood pressure, and cholesteral under control and monitored prior to any surgical procedure. Only proceed with elective surgery when you have medical clearance from your health care providers and follow their instructions to help ensure the best possible outcome.

Medical conditions do pose risks for surgical procedures. It all depends on how you manage those risks and if the benefits justify the risks.

Should I get a “mini” or a full “tummy tuck?”

This is a very commonly asked question, “which would be best for me?”

Well, the answer is complicated. Mini-tucks are appropriate procedures for patients with only a small amont of fat or excess skin below the belly button that hangs over their pant line or bikini bottom when they lean forward. This procedure is limited in the amount of stretch marks that can be remove and the amount of tightening that can be achieved. Often, liposuction can be added to this procedure to improve the results on the appropriate patient.

A full tummy-tuck, is a much more extensive procedure that aims to tighten and improve tone along the entire abdomen. This is achieved by elevating the skin up to the rib margin and then tightening the muscles along the middle all the way down to the pubis. The excess skin and fat are then trimmed off and the belly button repositioned to look natural. Depending on the patient and the amount of excess skin, we are often able to remove stretch marks all the way up to the belly button. This procedure has more down-time, and requires the use of internally placed drainage tubes to help with fluid build up. The results are generally superior to the mini-tuck in most patients.

Finally, I will also add, that not everyone is a candidate for these abdominal procedures. Some patients would not have a good result with either one depending on their body habitus and pre surgical weight or health.
